On February 3, 2025, an off-site review of the plan of correction was conducted. The plan of correction was accepted for all citations.

No evidence was provided regarding the manager's responsibilities to report and investigate suspected abuse, neglect, or exploitation.
R9-10-113 — The facility failed to provide evidence that staff with more than eight hours per week of direct resident interaction had documented freedom from infectious tuberculosis.
The manager did not ensure that there was a means of exiting the facility for residents without keys or special knowledge that allowed safe egress to an outside area at least 30 feet away and that alerts employees of egress.
The manager failed to ensure caregivers documented accidents, emergencies, or injuries including date, description, witnesses, actions taken, notifications, and prevention measures.
The manager did not ensure poisonous or toxic materials were stored in labeled containers in a locked area separate from food, dining, and medications, inaccessible to residents.

The following deficiencies were found during the on-site compliance inspection and investigation of complaint AZ00219363 conducted on December 4, 2024.
Findings
This inspection found five deficiencies related to failure to document an alleged abuse/neglect incident, lack of tuberculosis screening documentation for a caregiver, inadequate exit door alarm system, incomplete documentation of a resident accident, and improper storage of toxic materials.

The manager failed to document an alleged incident of abuse/neglect, including inaccurate timing and incomplete information about a resident's fall and injury, preventing assessment of immediate health and safety concerns.

The manager failed to ensure a means of exiting the facility that controlled or alerted employees of a resident's egress; a backyard door was unlocked and unalarmed, posing a risk of unmonitored resident exit.
The manager failed to ensure documentation of the time of a resident's accident and actions taken to prevent future incidents, resulting in inaccurate and incomplete incident records.
