Visit Reason
On 07/28/2025, the Office of the State Fire Marshal conducted an unannounced Fire and Life Safety Code inspection at Wesley Homes Lea Hill to determine compliance with applicable codes.

Findings
Multiple fire and life safety code violations were observed throughout the facility, including uncovered electrical junction boxes, improperly tethered gas appliances, missing grease caps on sprinkler heads, and lack of documentation for required inspections and maintenance. The facility was disapproved due to these deficiencies.

Deficiencies (18)
IFC 603.2.2 (2021) - The patio area had an uncovered electrical junction box, which is prohibited.
IFC 603.4 (2021) - The electrical closet had multiple items in front of electrical panels impeding the required 36-inch clearance.
IFC 606.4 (2021) - Gas fired wheeled appliances in the kitchen were not tethered to the wall as required.
IFC 701.5 (2018) - Electrical closet had penetrations in walls without fire resistance rated material installed.
IFC 705.2 (2021) - The facility was unable to provide documentation that the annual fire door inspection had been performed.
IFC 705.2.4 (2021) - The fire door near the dining area did not latch from a fully open position.
IFC 903.5 (2021) - The facility was unable to provide documentation for required sprinkler system testing and maintenance, and multiple sprinkler heads were loaded with dust or grease.
IFC 904.13.5.2 (2021) - The facility was unable to provide documentation for semi-annual kitchen suppression servicing, and sprinkler heads were missing grease caps or improperly oriented.
IFC 906.2 (2021) - The facility was unable to provide documentation for annual and monthly fire extinguisher servicing and inspections.

The Department completed a follow-up inspection of the Assisted Living Facility to verify correction of previously cited deficiencies and compliance with licensing requirements.

Findings
The follow-up inspection found no deficiencies and confirmed that the facility meets the Assisted Living Facility licensing requirements. All previously cited deficiencies were corrected.
