Visit Reason
The inspection was a biennial survey to assess compliance with regulations governing the licensing and functioning of Level III Residential Care Facilities and infection control.
Findings
The facility was found not in substantial compliance due to failure to provide current written consent for release of resident information for two residents and failure to provide evidence that unlicensed assistive personnel were trained by a registered nurse in diabetes management.
