Visit Reason
The inspection was an unannounced routine compliance check of Whisper Cove Assisted Living and MC to review adherence to licensing rules and regulations.
Findings
The inspection identified 15 rule noncompliances including deficiencies in personnel records, employee training, emergency and disaster preparedness, housekeeping, medication administration, resident rights, transfer and discharge procedures, and fire and disaster drills.
Deficiencies (13)
R432-270-9(9)(a)-(l) One employee did not receive all required in-service trainings including nutrition, medication assistance, and abuse reporting.
R432-270-9(14)(a-g) The licensee did not ensure the employee health program was fully followed, including health inventories and tuberculosis testing.
R432-270-10(2)(a)-(b) The licensee did not ensure each resident received all their legal rights upon admission in writing.
R432-270-12(2)(a)-(b) The licensee failed to serve a 30-day transfer notice to one resident and their responsible person.
R432-270-12(3)(a)-(g) The licensee did not ensure the transfer or discharge notice was in writing and placed in the resident file.
R432-270-21(3)(a)-(j) Personnel records lacked termination date, reason for leaving, health inventory, TB skin test documentation, and core competency training records for at least one employee.
R432-270-23(5) Cleaning agents, dangerous and/or flammable materials were not locked to prevent unauthorized access.
R432-270-23(6)(a-e) Housekeeping personnel did not receive all required training on cleaning procedures and safety.
R432-270-26(2)(a)-(c) The emergency and disaster response plan was not developed with state and local authorities and lacked notification procedures for the Silver Alert program.
R432-270-26(4)(a)-(k) The emergency and disaster response plan did not include plans for severe weather, missing residents, death of a resident, interruption of utilities, explosion, bomb threat, earthquake, windstorm, epidemic, or mass casualty.
R432-270-26(6)(a)-(j) The emergency plan did not include assignment of personnel to tasks, recruitment of additional help, or delivery of essential care under various emergency conditions.
R432-270-26(8)(a)-(d) Fire and disaster drills were not held the required number of times and lacked required documentation.
