Visit Reason
This is an unannounced fire and life safety re-inspection and re-certification survey conducted to determine compliance with applicable codes at White River Assisted Living.
Findings
The facility was found not in compliance at the time of inspection with multiple deficiencies cited related to electrical terminations, sprinkler system testing and maintenance, fire extinguishing system service, emergency power for illumination, and exit signage. Documentation for required testing and servicing was not provided, and some emergency lighting and exit signs were missing or not illuminated.
Deficiencies (6)
IFC 603.2.2 (2021) - Open junction boxes and open-wiring splices are prohibited. Approved covers must be provided for all switch and electrical outlet boxes. The kitchen's office has a missing receptacle cover.
IFC 903.5 (2021) - Sprinkler systems must be tested and maintained per Section 901. Documentation including annual sprinkler system report, annual forward flow test, and 5-year hydro testing was not provided.
IFC 904.13.5.2 (2021) - Automatic fire-extinguishing systems must be serviced at least every six months with certificates forwarded to the fire code official. First and second semi-annual servicing reports were not provided.
IFC 907.8 (2021) - Maintenance and testing schedules for fire alarm and detection systems must be maintained with records. Annual, semi-annual, and smoke detector sensitivity reports were not provided.
IFC 1008.3 (2021) - Power supply for means of egress illumination must be provided by the premises' electrical supply. Emergency lighting in the new building's southeast stairwell and stairwell by room 208 needs testing.
IFC 1013.1 (2021) - Exit and exit access doors must be marked by approved exit signs visible from any direction of egress travel. Two exit signs on the 2nd floor in the old building are not illuminated, and an exit sign is missing on the 1st floor by room 116.

Findings
The fire alarm panel was found to be in trouble status and the communication radio network was causing false trouble signals. The facility did not follow its fire watch policy correctly, performing 30-minute intervals instead of the required 15-minute intervals. The fire alarm panel network remains in trouble and may not communicate properly.

IFC 901.7 (2021) - The facility failed to provide fire watch patrols at 15-minute intervals during fire alarm system trouble, instead performing them every 30 minutes. The fire department and fire code official were not properly notified as required.

IFC 310.7 2021 - Lighted matches, cigarettes, cigars, or other burning objects shall not be discarded in a manner that could cause ignition of combustible material.
IFC 606.3.3.3 2021 - Records for inspections and cleanings shall be completed and maintained after each inspection or cleaning.
IFC 701.6 2018 WAC 51-54A - The owner shall maintain an inventory of all required fire-resistance-rated construction and ensure proper inspection and repair.
IFC 705.2 2021 - Opening protectives in fire-resistance-rated assemblies shall be inspected and maintained in accordance with NFPA 80 and NFPA 105; fire doors and smoke and draft control doors shall not be blocked or obstructed.
IFC 903.5 2021 - Sprinkler systems shall be tested and maintained in accordance with Section 901.
IFC 904.13.5.2 2021 - Automatic fire-extinguishing systems shall be serviced at least every six months and after activation, with inspection certificates forwarded to the fire code official.
IFC 907.8 2021 - Fire alarm and detection systems shall be maintained with records of inspection, testing, and maintenance.
IFC 907.8.3 2021 - Smoke detector sensitivity shall be checked within one year after installation and every alternate year thereafter, with calibration tests performed as required.
NFPA Standard 10 Section 6.2.1 - Fire extinguishers shall be inspected initially and at approximately 30-day intervals, manually or electronically monitored as needed.

Complaint Details
Complaint #176215 involved a sprinkler system issue caused by an elevator technician cutting the water line. The fire department responded to the fire alarm panel activation. No IFC violations were found, indicating the complaint was unsubstantiated.
Findings
The elevator technician accidentally cut the water line to the east portion of the community, causing a water shutoff for about 4 hours. The facility was on fire watch every 30 minutes until repairs and fire panel reset were completed. No fire code violations were observed.

IFC 706.1 2018 - Dampers protecting ducts and air transfer openings shall be inspected and maintained; damaged products shall be repaired or replaced. The facility's fire damper report showed one damper failed and documentation was not provided showing correction.
IFC 903.5 2009, 2012, 2015, 2018 - Sprinkler systems shall be tested and maintained in accordance with Section 901. The facility was unable to provide documentation for quarterly sprinkler reports and forward flow test.
