Visit Reason
Biennial state compliance review of the assisted living residence by the Massachusetts Executive Office of Aging & Independence (AGE) on September 4, 2024.
Findings
The state cited 5 findings, 4 of which were repeats from the previous review. The action taken was a plan of correction required. The number of findings is below the state median of 6.

Incident reporting: The Residence filed 23 incident reports greater than 24 hours after the occurrence of the incident during the period reviewed (repeat finding).
Emergency call response: In the months of October 2022, August 2023 and July 2024 there were 125 e-call response times over the ten minutes required by the Residence policy, and the Residence is not consistently monitoring the reasons for late e-call response times (repeat finding).
Quality assurance program: Documentation of staff responsible for follow-up was missing in 2022 and 2023 calendar years, and the Residence was missing staff responsible for follow-up in calendar years 2023 and 2024 (repeat finding).
Resident screening and assessment: Five records were missing documentation of the Legal Representative and the scope of authority (repeat finding).
Memory care unit: Chemicals were not secured in the common kitchen area in the Special Care Unit, and exit doors in the common areas within the Special Care Residence were not secured with alarms at the time of the Compliance Review.
