The inspection record traces a clear trajectory. August 2023 brought a serious finding: state surveyors investigated complaints of resident falls with injuries. The investigation substantiated that the facility had failed to maintain updated care plans with documented fall prevention measures for residents with fall history. One fall resulted in a resident death.
