The Gardens at Quail Springs Assisted and Memory is a 133-bed assisted living and memory care center at 14300 North Portland Avenue, Oklahoma City, Oklahoma County, 10.7 miles from downtown. The facility is licensed by the Oklahoma State Board of Health and offers assisted living and memory care with 24-hour staffing. Current occupancy is 68 residents from a 133-bed capacity (51 percent). Nursing hours averaged 28 percent above the Oklahoma state benchmark.
Amenities include 24/7 on-site team, all utilities included, housekeeping and laundry, three meals daily, medication management, bathing and grooming assistance, scheduled transportation, activities and wellness programming, and apartment maintenance. Private pay; Medicaid and Medicare not accepted.
The regulatory record reflects substantiated findings and multiple deficiency clusters requiring attention. Over seven years, The Gardens at Quail Springs logged 11 deficiencies, averaging 1.6 annually, 67 percent better than Oklahoma’s 4.9-deficiency benchmark.
However, serious incidents and deficiencies reveal patterns warranting scrutiny. An August 31, 2022, complaint investigation substantiated physical abuse through use of restraints on two wandering residents. Deficiencies included failure to ensure individualized care plans and interventions for wandering, failure to prevent abuse through improper restraint use, failure to complete thorough abuse investigation, delayed abuse reporting (three days late), and failure to timely report the abuse allegation within one business day.
A December 16, 2022, revisit found the facility in substantial compliance after corrective actions. A June 6, 2023, state licensure survey and complaint investigation cited three deficiencies: use of unpasteurized eggs served to residents, failure to ensure resident assessments were coordinated and signed by a registered nurse or physician for 10 of 10 sampled residents, and failure to maintain resident assessments for five years for 5 of 10 residents reviewed.
A June 21, 2024, complaint investigation found no deficient practice related to physical abuse allegations but identified two deficiencies: failure to submit incident reports to Oklahoma State Department of Health for two residents requiring reporting and failure to maintain organized, accurate clinical records including physician notification of incidents.
An April 4, 2025, complaint investigation found the facility providing ADLs per regulation but cited one deficiency for failure to notify the department of a change in administrator. A July 8, 2025, complaint investigation addressed allegations of failure to ensure food temperature, call light response, resident choice, elopement prevention, and infection control.
Two residents eloped during a fire alarm event, creating an immediate jeopardy situation due to inadequate supervision. The facility was found in compliance with food, call light, and resident choice allegations. One deficiency was cited for failure to provide supervision to prevent elopement; the immediate jeopardy was resolved after corrective actions.
No fines or license suspensions are recorded. The facility’s pattern reflects recurring deficiencies in supervision, incident documentation and reporting, clinical record maintenance, and care plan individualization across multiple years and complaint investigations.

Operated by Saint Ann Retirement Center, Inc., and licensed by the Oklahoma State Board of Health, the facility offers assisted living, independent living, memory care, respite care, and rehabilitation services with 24-hour staffing. Current occupancy is 31 residents from a 40-bed capacity (78 percent). Nursing hours averaged 28 percent above the Oklahoma state benchmark.
Amenities include beautifully appointed common areas, library, game room, wellness programs, cultural activities, beauty and barber salon, restaurant-style meals, concierge services, and scheduled transportation. Programs include life enrichment activities, religious services, and transportation. Private pay; Medicaid and Medicare not accepted.
The regulatory record reflects strong compliance over six years with one critical deficiency in 2025. Over six years, Saint Ann logged one deficiency, averaging 0.2 annually, 96 percent better than Oklahoma’s 4.9-deficiency benchmark. Relicensure surveys on June 27, 2019, and August 2, 2019, found no deficiencies. An August 18, 2020, COVID-19 special focus infection control survey found no deficiencies.

A May 25, 2022, complaint investigation addressing allegations of failure to ensure licensed personnel administered medications found the complaint unsubstantiated with no deficiencies. However, a June 4, 2025, complaint investigation substantiated one critical deficiency: the facility failed to ensure medication was administered per physician orders for one resident, resulting in two missed doses. The medication error likely contributed to the resident’s disorientation and elopement from the unlocked facility. A June 20, 2025, license renewal authorized continued operation with a maximum capacity of 40 beds. The facility’s occupancy has declined from 46 residents in June 2019 to 31 residents in June 2025.
No fines or license suspensions are recorded.
