The regulatory history reflects recurring compliance failures across medication administration, clinical documentation, and staffing adequacy. Since 2019, the facility accumulated 34 deficiencies at 4.9 annually. The most recent inspection on October 9, 2025, cited two deficiencies: two of five sampled direct care staff lacked training in first aid and CPR, and one of eleven sampled residents had inaccurate clinical records. A complaint investigation on April 20, 2025, identified deficiencies in admission and annual resident assessments and failure to maintain minimum staffing on one of three shifts observed; corrections were verified effective May 18, 2025.
The June 28, 2024, renewal inspection found twelve deficiencies spanning individualized service plan documentation gaps, missing assessment signatures, hot water exceeding safe limits at 121 to 140 degrees Fahrenheit, inadequate medication review frequency by nurses, memory care staff lacking dementia training, incomplete service contracts, delayed criminal background checks, and one observed failure to administer medication as prescribed. The May 2023 complaint investigation substantiated eight deficiencies including kitchen and food safety violations, systematic medication administration failures numbering 26 errors across sampled residents, falsified medication records, failure to report alleged misappropriation to the state agency, and inadequate care coordination with home health services. A May 2019 complaint investigation found deficiencies in abuse prevention policy implementation and failure to document staff training. Complaint investigations in July and October 2024 and February 2022 found no deficiencies.
The pattern shows documented corrections to some prior deficiencies but persistent recurrence of medication, assessment, documentation, and training gaps despite multiple corrective action plans. No fines or enforcement actions have been recorded.
Direct inquiry is necessary regarding the corrective actions taken following the October 2025 deficiencies in first aid/CPR training and clinical record accuracy, the June 2024 deficiencies in medication review protocols and hot water safety, and systemic changes implemented to prevent recurrence of medication administration errors and documentation failures documented in May 2023.

Operated by JCMH Health Care Corporation and licensed by the Oklahoma State Board of Health, the community offers assisted living and memory care with 24-hour certified staff. Current occupancy is 47 residents from a 68-bed capacity (69 percent). Nursing hours averaged 28 percent above the Oklahoma state benchmark.
Amenities include group activities, wireless call systems, comfortable community areas, security systems, covered parking, 24-hour certified staff, and transportation for medical and dental appointments, shopping, errands, and social events. Private pay; Medicaid and Medicare not accepted.
The regulatory record reflects strong compliance with periodic corrections. Over six years, Tamarack logged five deficiencies, averaging 0.8 annually, 84 percent better than Oklahoma’s 4.9-deficiency benchmark.
An October 16, 2019, relicensure survey cited four deficiencies: registered nurse failed to ensure accuracy of monthly medication reviews and staff delegated to perform monthly medication counts for two sampled residents, the facility failed to ensure an antibiotic was available for one of two sampled residents with physician orders, the facility failed to maintain an individual inventory record for a schedule II medication for one sampled resident, and the facility failed to ensure resident rights related to medical care. These deficiencies represented potential for more than minimal harm.
A revisit on January 30, 2020, confirmed correction. A July 29, 2020, COVID-19 special focus infection control survey found no deficiencies. Relicensure surveys on August 18, 2020, November 10, 2022, and June 20, 2025, found no deficiencies.
A February 28, 2024, licensure survey with complaint investigation found one deficiency: the facility failed to report required incidents to the Oklahoma State Department of Health within one business day for five of twelve sampled residents. The Administrator acknowledged late reporting and lack of a reporting policy.
The facility’s occupancy declined from 51 residents in October 2019 to 47 residents in February 2024. No fines or license suspensions are recorded.

The regulatory record reflects perfect compliance across all inspections since initial licensure. Over four years, Sooner Station logged zero deficiencies. An initial licensing inspection on June 21, 2022, found no deficiencies. A license renewal on December 13, 2022, authorized operation with a maximum capacity of 100 beds.
