Current occupancy is 56% (84 of 150 beds), considerably below Nevada’s 81.5% average. The community holds a BBB A+ rating (not accredited) and caters exclusively to private-pay residents.

Inspection records over 12 years reveal 49 deficiencies across 40 surveys; 21% fewer than the Nevada average, though the inspection frequency itself is 90% higher than state norm, reflecting intensive regulatory oversight.

The June 2025 re-grading survey (most recent) found three employee file deficiencies: missing TB testing (1 of 5 employees), lapsed CPR certification (1 of 5), and incomplete infection control training (2 of 5). Four complaints were investigated; three proved unsubstantiated, one substantiated without regulatory citation. A January 2025 annual inspection assigned a C grade with seven deficiencies spanning employee health screening gaps (TB tests, CPR training, cultural competency), kitchen sanitation failures (pooling water, non-operational sinks, staining, debris, ice in freezers, peeling walls), resident care lapses (missing annual physicals, medication dosage errors, inadequate infection control training for aides), and unsafe dementia unit operations.

Substantiated complaints documented across prior inspections reveal concerning patterns: residents told to purchase their own toilet paper due to budget constraints (June 2024), delayed discharge of a resident after significant condition decline (August 2023), theft of resident property with deficient grievance procedures (August 2022), and delayed response to resident call bells (May 2022). Kitchen sanitation violations recur repeatedly; unsecured medications in resident rooms and oxygen tanks have appeared across multiple cycles.

The facility incurred $5,500 in federal fines (January and March 2022) with no license suspension.

Deficiency-per-inspection rate (1.2) is 60% better than Nevada average despite elevated frequency of surveys.
