Licensed by the Minnesota Department of Health, Health Regulation Division as an assisted living facility approved for dementia care, the facility comprises studio, one-bedroom, and two-bedroom apartments (451 to 1,348 square feet) in a mid-rise building.

Current occupancy is seventy-five percent. The community offers a full-service restaurant with seasonal menus, dedicated concierge, wellness center, spa and salon, therapy and rehabilitation services, enhanced care, transportation, fitness and recreation, and recreational and educational programs.

The Minnesota Department of Health, Health Regulation Division conducted an annual inspection on October 10, 2024 (census eighty-three, fifty-nine residents in dementia care), identifying six deficiencies.

Food preparation was non-compliant with Minnesota Food Code at widespread scope. The contract included language waiving facility liability for resident health, safety, or personal property at widespread scope. Incomplete annual staff training for one employee lacked required person-centered planning and service delivery topics. Medication prescriptions for one resident were incomplete, lacking dose and directions for use.

Medication disposition records for one discharged resident were missing prescription numbers. One staff member administered medication without measuring the correct dose of topical medication.

Time periods for correction ranged from seven to twenty-one days. No complaint investigations or enforcement actions were documented.
