The June 28, 2024, renewal inspection found twelve deficiencies spanning individualized service plan documentation gaps, missing assessment signatures, hot water exceeding safe limits at 121 to 140 degrees Fahrenheit, inadequate medication review frequency by nurses, memory care staff lacking dementia training, incomplete service contracts, delayed criminal background checks, and one observed failure to administer medication as prescribed. The May 2023 complaint investigation substantiated eight deficiencies including kitchen and food safety violations, systematic medication administration failures numbering 26 errors across sampled residents, falsified medication records, failure to report alleged misappropriation to the state agency, and inadequate care coordination with home health services. A May 2019 complaint investigation found deficiencies in abuse prevention policy implementation and failure to document staff training. Complaint investigations in July and October 2024 and February 2022 found no deficiencies.

The pattern shows documented corrections to some prior deficiencies but persistent recurrence of medication, assessment, documentation, and training gaps despite multiple corrective action plans. No fines or enforcement actions have been recorded.

Direct inquiry is necessary regarding the corrective actions taken following the October 2025 deficiencies in first aid/CPR training and clinical record accuracy, the June 2024 deficiencies in medication review protocols and hot water safety, and systemic changes implemented to prevent recurrence of medication administration errors and documentation failures documented in May 2023.

The facility was found in compliance with all regulations, with call lights within reach and answered timely, clean and comfortable environment, appropriate staff assistance, accurate medication administration, and safety checks completed every two hours.

The facility’s occupancy has grown from 88 beds in June 2022 to 92 residents in a 100-bed capacity in January 2026. No fines or license suspensions are recorded.
