Garden Ridge Senior Living operates as a large-scale community in Peoria at 18170 N 91St Ave: 259 beds across independent living, assisted living, and memory care. RIDGES AT PEORIA, LLC owns it; Holly Vandermate administers, with George Bakhit leading operations. The building is two stories with one and two-bedroom units (760 to 1,034 square feet). The facility’s been open 6 years.
Walk Score 61 means some walkable errands. Downtown Peoria is 6.7 miles away; the nearest hospital is 3.21 miles.
Staffing runs 24 hours with licensed nurses, CNAs, and a 24/7 on-call RN. Physician visits and wellness checks happen regularly. Medication supervision and daily living assistance cover the expected range: dressing, bathing, hygiene, mobility. The property lists amenities comprehensively: garden, housekeeping, laundry, wellness program, library, courtyards, recreational room, outdoor pool, patio, covered parking.
Meals are three homemade daily plus snacks. Pets are allowed. Transportation services are available.
Over three years, 7 complaints filed (17% above Arizona average) with 2.3 per year (15% above state average). The facility triggered 12 total inspections; 140% above the Arizona norm.
The most recent complaint found a deficiency for “failing to ensure resident dignity and respect.” That’s not a narrow compliance issue; that’s a care culture question. A prior annual inspection cited four deficiencies: uncertified caregiver, pet licensing and vaccination failures. An off-site level-of-care inspection found no problems.
Garden Ridge is a newer facility in a competitive market with elevated complaint activity and inspections above normal. The dignity-and-respect deficiency combined with staffing certification gaps suggests basic operational discipline issues that extend beyond a single audit.

Regulatory compliance has been a persistent challenge. Since 2022, the facility has accumulated 14 deficiencies across 7 inspections and 6 complaint investigations, averaging 3.5 deficiencies per year (5 percent below Arizona’s 3.7 per-year average). However, the facility faces repeated violations and enforcement actions. Emergency responder documentation has emerged as a critical and recurring deficiency.
An August 22, 2025 complaint investigation identified five deficiencies including emergency responder documentation failures, administration notifications, and residency agreement deficiencies. This same issue reappeared in subsequent enforcement actions: December 2, 2025 ($250 fine, pending) specifically documented the facility’s failure to provide emergency responder information-a repeat deficiency from prior inspections-and January 2, 2026 ($250 fine, paid in full). Earlier enforcement actions in 2024 included $500 fines (one paid, one pending as of the latest reports) for premises safety concerns.
Additional documented deficiencies span caregiver skills verification, medical records safeguards, disaster plan review, fire inspection compliance, and fall prevention training. The inspection summary describes ongoing challenges with documentation and safety compliance with no clear trajectory of improvement. No license suspensions or revocations have been issued.
Located 20.5 miles from downtown Phoenix and 3.66 miles from the nearest hospital, the facility sits in an area with a walk score of 27 (somewhat walkable; most trips require transportation).
Families should ask about corrections for repeated emergency responder documentation violations, verify emergency protocols, and request recent inspection documents from the Arizona Department of Health Services, Bureau of Long Term Care Licensing.
