A serious medication error occurred during a September 11, 2024 complaint investigation. This deficiency was cited as a failure to ensure residents were free from significant medication errors and resulted in a moderate citation marked as corrected. A December 2022 inspection documented failure to assist a resident with grooming and personal hygiene.
