On-site complaint investigation and annual compliance survey at a Hospice Service Agency conducted 6 November 2024.

Complaint Details
The following deficiencies were found during the on-site State Compliance and Complaint survey conducted November 6, 2024. Based on the rules found at R9-10 Article 6, Hospices, the Department has authorized the facility to continue providing the following Scope of Service: Hospices.
Findings
The inspection found multiple deficiencies related to administration, care planning, patient rights, and nursing services. The facility failed to implement policies and procedures adequately, review and update care plans, inform patients of hospice service charges, and ensure nursing services were provided according to policies.
