This Plan of Correction responds to the IDPH Annual Licensure Survey conducted on 10/15/22.
Findings
The facility committed to posting evacuation maps in lobby areas, conducting quarterly checks for evacuation map postings, scheduling CPR training for all assisted living associates, ongoing CPR certification checks, and re-inservicing nurses to identify and report incidents timely.

The establishment failed to document residents requiring assistance during fire and tornado drills and did not post evacuation route maps. The facility also failed to ensure at least one CPR-certified staff member was working on the night shift.
