Summit Home Care and Hospice runs out of an office in Columbus’s Gateway neighborhood, about 4.4 miles from downtown, and everything they do actually happens somewhere else: in patients’ own homes across the region. Walk Score for the office area comes in at 39, somewhat walkable, though that’s mostly irrelevant since staff are the ones driving out to patients, not the other way around. Services here are for adults 65 and up.

The clinical side is built around a doctor on staff plus licensed nurses and CNAs, and rehabilitation specifically covers orthopedic, cardiopulmonary, and neurological conditions. Before any of that starts, there’s a free in-home assessment to actually figure out what a given patient needs. Fall prevention and emergency response round out the day-to-day safety side of things.

Then there’s the hospice piece, which is a genuinely different kind of work. For patients with six months or less of life expectancy, the hospice team handles pain control, symptom relief, and spiritual support, the things that matter most when recovery isn’t the goal anymore. A companion Volunteer Program sits alongside that clinical care, extra presence and support for patients and families during a stretch of time that’s hard no matter what.

Medicare, Medicaid, and private pay are all accepted, which covers most of the ways people actually pay for this kind of care. What Summit offers is a genuine range: rehabilitation for people working toward recovery, and hospice for people who aren’t, both delivered at home instead of in a facility. That range, recovery care on one end and end-of-life care on the other, is the actual shape of what this agency does.
