Findings
No deficiencies were found during this inspection. The facility was found to be in compliance with all applicable federal, state, and local emergency preparedness requirements.

Visit Reason
On-site annual compliance inspection of AZORNA HOSPICE AND PALLIATIVE CARE conducted August 2 through August 4, 2023 using the Hospice Service Agency worksheet.

Findings
Four deficiencies were cited related to failure to provide employee orientation on patient rights and complaint filing procedures. Two deficiencies were resolved by an acceptable Plan of Correction submitted on November 13, 2023, with no follow-up survey planned.

R9-10-603 — Employees #1 through #10 failed to receive orientation training on policies and procedures covering patient rights, risking care that does not honor patient preferences during end of life.
R9-10-603 — The Administrator failed to provide employees orientation on how to file a complaint on behalf of a patient, increasing risk of harm and abuse in unsafe situations.
R9-10-603 — An acceptable Plan of Correction was submitted on 11/13/2023; no follow-up onsite survey will be conducted for this event.
