Visit Reason
On-site complaint investigation of intakes 00156966, 00156562, 00146276, and 00143149 at a Hospice Service Agency, conducted 12-13 February 2026.

Complaint Details
The unannounced, on-site State Complaint Investigation Survey was conducted pursuant to Arizona Administrative Code Title 9, Chapter 10, Article 6: Hospices, for intakes 00156966, 00156562, 00146276, and 00143149.
Findings
The inspection found four deficiencies related to tuberculosis screening documentation, incident reporting after a patient fall, implementation of a fall prevention and recovery program, and failure to provide services according to a patient's care plan. Corrective plans were provided for all deficiencies.

Deficiencies (4)
R9-10-113 — The Administrator failed to ensure that tuberculosis exposure risk assessments were signed by a medical practitioner, occupational health provider, or local health agency, resulting in missing authentication of employee freedom from infectious tuberculosis symptoms.
R9-10-604 — The Administrator failed to ensure staff implemented the incident reporting policy after a patient fall, resulting in two staff witnessing a fall with injury but not completing incident reports or documenting the event in the patient’s medical record.
R9-10-606 — The Administrator failed to ensure agency staff implemented the fall prevention and recovery program, resulting in a patient not receiving a post-fall assessment by a registered nurse until seven days after a fall with injuries.
R9-10-608 — The Administrator failed to ensure services were provided according to the patient’s care plan, resulting in a patient fall and injuries when two staff did not follow the updated plan requiring use of a Hoyer lift with two-person assist.
