On-site compliance (annual) inspection at an Assisted Living Home conducted on April 18, 2025.

Findings
Two deficiencies were found related to outdated service plans and incomplete disaster drill documentation. Plans of correction were provided for both deficiencies.

Deficiencies (2)
R9-10-808 — The manager failed to ensure that one resident's service plan was updated every six months as required, with the last update incorrectly dated June 30, 2025 instead of June 30, 2024. This posed a risk due to lack of current direction for personal care services.
R9-10-818 — The manager failed to ensure that disaster drills for employees were conducted and documented on each shift at least once every three months, with the last drill conducted on December 4, 2024. This posed a risk if employees were unable to implement the disaster plan.

Findings
The inspection found four deficiencies related to staff training, personnel records, and medication administration. Plans of correction were provided for all deficiencies.

Deficiencies (4)
36-420.01 — The facility failed to administer a fall prevention and fall recovery training program for all staff as required by their documented program, including missing current annual training for one employee.
The manager failed to ensure cardiopulmonary resuscitation training included a demonstration of the employee's ability to perform CPR for one of two sampled employees.
The manager failed to ensure a personnel record included documentation of compliance with A.R.S. § 36-411(C)(1) regarding contacting previous employers for one of two employees sampled.
The manager failed to ensure medication administered to a resident was in compliance with a medication order, including missing orders for two medications and incorrect dosage for another.
