Emergency call response: In April 2023 there were 23 emergency call response times over the 10 minutes required by the Residence policy.

Optional services: Two records were intermittently missing staff signature documentation of the nurse administering medications.

Resident screening and assessment · Resident care plans: Two records were missing documentation of a reassessment review every six months, three service plan records lacked documentation of individualized enrichment activities, and documentation confirming residents could independently navigate around bed rails was missing for seven residents in 2023.

Quality assurance program: Documentation was insufficient to support that a Resident, Resident Representative, or legal guardian provided informed consent for medication administration in a common area, and the LMA policy lacked procedures for administering medication in a common area.

Incident reporting: The Residence filed 25 incident reports more than 24 hours after the occurrence of the incident during the period reviewed.
