Renewal inspection of the home for the aged license.
Findings
The home was found to be in non-compliance with 6 rules; a written corrective action plan was required before the license is renewed.

R 325.1922 Admission and retention of residents: Resident A moved into the facility on 12/12/23 but her tuberculosis screening was not completed until 1/4/24, after admission.
R 325.1923 Employee's health: Employee 1 did not have evidence that initial tuberculosis testing was completed within 10 days of hire; hire date was 9/27/24 but TB test on file was from 5/4/24.
R 325.1932 Resident medications: Resident A missed three doses of oxybutynin due to medication unavailability and documentation errors; Resident B missed multiple doses of vitamins with delayed family notification.
R 325.1954 Meal and food records: Meal census records were combined with another building and included meal counts for residents outside this license. Repeat violation established.
R 325.1968 Toilet and bathing facilities: The communal bathroom in the “C” wing was being used for storage and housekeeping purposes.
R 325.1976 Kitchen and dietary: Multiple perishable food items in the commercial kitchen’s refrigerator and freezer were left uncovered and unlabeled. Repeat violation established.

Findings
The licensee legally changed its name from Alterra Healthcare Corporation to Brookdale Senior Living Communities, Inc. This is only a change in the legal entity name and the tax identification number remains the same.
