Findings
The facility failed to provide required annual inspections, testing, and maintenance reports for fire resistance-rated construction, fire doors, fire dampers, sprinkler systems, carbon monoxide detectors, emergency lighting, and fire drills. The overall status is Disapproved due to multiple deficiencies.

IFC 701.6 (2021) - The facility failed to provide an annual inspection of fire resistance rated construction as required by code.
IFC 705.2 (2021) - The facility failed to provide an annual fire door inspection report as required by code.
