Visit Reason
Annual Licensure Survey conducted with violations cited in multiple sections including disaster preparedness, personnel requirements, employee orientation, health evaluations, background checks, license requirements, and service plans.
Findings
The establishment was found to have multiple violations including failure to evacuate residents during fire drills, lack of CPR certification coverage 24/7, incomplete new employee orientation, missing initial health evaluations and TB testing for employees, failure to comply with Health Care Worker Background Check Act, failure to apply for license renewal prior to expiration, and omission of occupational therapy in a resident's service plan.
