Findings
The most recent inspection in April 2024 cited 7 violations of state standards. Previous inspections, including a 2019 life safety code survey, identified multiple corrected deficiencies related to fire safety and facility maintenance. An enforcement action in 2023 involved an administrative penalty for failure to ensure resident protection from abuse, neglect, and exploitation.

The facility failed to provide a manual fire alarm pull within five feet of each exit door with a sign stating, "Pull to release door in emergency."
The facility failed to ensure an emergency generator was in compliance with licensing standards for assisted living facilities.
The facility failed to ensure the required sprinkler system was inspected, tested, and maintained in compliance with NFPA 25.
The facility failed to provide the required sprinkler system to meet NFPA 13.
