The Office of the State Fire Marshal conducted a scheduled fire protection inspection at the facility.
Findings
The facility was found to have multiple deficiencies related to fire safety equipment, maintenance, and required documentation. The facility failed to provide several required inspection reports and had physical obstructions and missing covers. The overall approval status was Disapproved.
Deficiencies (11)
IFC 315.2.1 (2021) - Fire sprinkler heads in downstairs areas were obstructed due to insufficient ceiling clearance in nonsprinklered areas. Storage clearance requirements were not met.
IFC 603.2.2 (2021) - Hallway electrical cover was missing outside room 1 in the hallway and an electrical cover plate was missing in the head pump closet.
IFC 701.6 (2021) - The facility failed to provide the annual inspection report for fire resistance rated construction.
IFC 705.2 (2021) - The facility failed to provide the annual fire door inspection report as required for inspection and maintenance of fire-resistance-rated assemblies.
IFC 903.5 (2021) - The facility failed to provide annual fire sprinkler inspection, annual forward flow testing, and quarterly fire sprinkler inspection reports.
IFC 906.2 (2021) - The facility failed to provide the annual fire extinguisher inspection report.
IFC 0915.1 (2021) WAC 51-54A - The facility failed to provide monthly carbon monoxide detector testing reports and a smoke detector was missing in room 8.

Visit Reason
The inspection was a scheduled fire and life safety code inspection conducted by the Office of the State Fire Marshal at Windsor Gardens Memory Care.
Findings
The facility was found to have multiple deficiencies related to fire sprinkler testing, fire alarm inspection, carbon monoxide detector testing, emergency lighting, and fire drills. All violations remain uncorrected, resulting in a disapproved status.
Deficiencies (6)
IFC 903.5 2021 - Sprinkler systems were not tested and maintained as required. Facility failed to provide annual fire sprinkler inspection, annual forward flow, and quarterly fire sprinkler inspection reports. Fire sprinkler trim ring missing in stairwell.
IFC 907.8 2021 - Fire alarm and detection systems were not maintained and tested per code. Facility failed to provide annual fire alarm inspection. Fire alarm found to have dialer 2 issue.
IFC 915.6 2021 WAC - Carbon monoxide alarms and detection systems were not maintained as required. Facility failed to provide monthly carbon monoxide detector testing. Fire alarm found to have dialer 2 issue.
IFC 1031.10.1 2018 - Emergency lighting equipment was not tested monthly as required. Facility failed monthly emergency light testing.
IFC 1031.10.2 2018 - Battery-powered emergency lighting equipment was not tested annually for battery power duration. Facility failed to provide annual emergency light testing.
WAC 212-12-044 - Fire drills were not conducted as required. Facility failed to provide fire drills as required for Group I, E, and R2 occupancies.

The Department completed a follow-up inspection of the Assisted Living Facility to verify correction of previously cited deficiencies.
Findings
The follow-up inspection on 04/30/2024 found no deficiencies and confirmed that the facility meets Assisted Living Facility licensing requirements.
