Almond Heights, a 740‑type group home with a capacity of 145 residents, was the subject of three unannounced inspections in mid‑2021. The first visit on May 26, 2021 addressed a broad range of complaints that had been filed in September 2020, including resident injuries, medication errors, delayed response to calls, and concerns about food quality and missing belongings. All allegations were deemed unsubstantiated or unfounded.

A second inspection on July 8, 2021 followed complaints of disrepair and the absence of a specialized diabetic menu. While one of sixteen air conditioners was in the process of replacement, the evaluator noted that the facility had maintained comfortable temperatures and documented timely repairs. A review of the weekly cycle menu revealed that diabetic desserts were available, and no deficiency in menu options was substantiated. The claim that the facility was in disrepair was also ruled unsubstantiated. The same visit addressed concerns that staff were slow to respond to resident calls; interviews and documentation revealed that staff promptly checked residents and had instituted frequent checks for those with limited mobility. This allegation was likewise found to be unsubstantiated.

Across all investigations, a consistent pattern emerged: complaints were raised, but the evaluator’s evidence‑based review revealed that Almond Heights complied with state licensing requirements and maintained effective communication, training, and safety protocols. The facility demonstrated a willingness to document and correct any identified concerns, and no violations were substantiated. These findings underscore the importance of rigorous record‑keeping and proactive staff training in maintaining compliance and resident safety in community care settings.
