Inspection Report Summary for Angels Nursing Center

Redwood Senior Living Bakersfield’s early 2021 licensing activities—including a pre‑licensing televisit and a COMP II telephone call—were completed without identified deficiencies, and the facility’s initial census was recorded as 40 of its 41‑bed capacity. Those visits confirmed basic standards of care, temperature control, cleanliness, and the presence of required safety equipment.

In late 2021, however, two unannounced case‑management visits uncovered compliance gaps. The first, conducted on 8 June 2021, involved an alleged AWOL incident. While the inspector was unable to obtain conclusive evidence at that time, the report noted that further investigation was warranted. The second visit, on 4 November 2021, revealed that the facility had admitted and retained a resident who required total assistance with all activities of daily living—a prohibited health condition under Section 87615(a)(5) of Title 22. The facility received a citation and a plan of correction with a due date of 8 November 2021.

A 25 June 2024 complaint investigation substantiated allegations that staff mismanaged residents’ medications. Review of the Centrally Stored Medication and Destruction Record and the Medication Administration Record showed inaccurate documentation for at least one resident. The investigation cited CCR 87465(h)(6)(C)(D), requiring the licensee to implement daily accountability checks, train medication technicians, and provide evidence of completed training by the stated deadline.

These findings illustrate a pattern of challenges in medication documentation and resident eligibility that surfaced after the facility’s initial compliance period. While the 2021 visits found no issues during the pre‑licensing stage, subsequent inspections have highlighted recurring deficiencies related to Title 22 regulations. The corrective actions outlined in the 2024 report will determine whether Redwood Senior Living Bakersfield can maintain its license without further civil penalties.
