Astoria Park Senior Living Facility (Facility #198603566) was the subject of two separate Community Care Licensing Department investigations within 2023. In a May 13, 2023 visit, evaluator Mary Flores reviewed a complaint alleging that a resident (R1) had been struck by an object, sustaining a sternal fracture. Although the resident was hospitalized and the incident was reported to the police, an examination of video footage, staff interviews, and medical documentation yielded no corroborating evidence of an assault. The investigation concluded that the preponderance of evidence standard was not met, and the allegation was declared unsubstantiated.
A subsequent investigation, conducted on October 19, 2023, addressed complaints that staff had failed to provide adequate care and supervision and had blocked the exit door of the memory‑care unit. Interviews with residents and staff revealed that multiple residents had left the unit unsupervised, and evidence of a sliding wood board that obstructed the exit door was observed. The investigator noted that the facility’s care plans and staffing levels did not adequately address the wandering behaviors of residents, and the egress system was not fully functional. The findings were substantiated, and the Department cited specific regulatory deficiencies under CCR 87705(k)(5) and CCR 87303(a). A plan of correction was issued, requiring the administrator to develop a comprehensive care plan for the memory‑care unit and to ensure that doors remain operable and safe within 10 days.
The two investigations highlight a pattern of concerns related to resident safety and supervisory practices at Astoria Park. While an alleged assault could not be proven, the facility’s failure to manage wandering behaviors and maintain safe exit routes has been documented. The substantiated findings underscore the need for immediate corrective action to safeguard residents and to align facility operations with California’s health and safety regulations.
