Visit Reason
Unannounced routine inspection of Autumn Park Assisted Living II to assess compliance with state assisted living facility regulations.
Findings
The inspection identified multiple deficiencies including missing employee training and documentation, incomplete resident service plans, unsecured hazardous materials, lack of fire and disaster drills in 2024, and deficiencies in emergency preparedness and maintenance.

R432-1-4: Two direct care employees were observed to not be wearing name badges.
R432-270-8(1): Significant change logs did not include the facility's action or response.
R432-270-8(2): The licensee did not have a written and signed job description on file at the facility.
R432-270-9(7)(a)-(f): Two employees did not have all of the required orientation trainings.
R432-270-9(9)(a)-(l): Two employees did not have all required in-service trainings.
R432-270-9(14)(a-g): The licensee did not ensure that 1 employee had a health inventory and 2 employees had TB skin-tests.
R432-270-10(2)(a)-(b): Five admission agreements did not include that the resident may file a complaint with the state long-term care ombudsman.
R432-270-11(9)(a)-(c): Two resident service plans did not include hospice services.
R432-270-14(2): Two resident assessments were not used to develop the service plan.
R432-270-14(3)(a)-(e): Five service plans did not include the services provided, who will provide the services, how the services were provided, and the frequency of services.
R432-270-15(1): There was no written policy for defining the level of nursing services provided by the facility.
R432-270-23(5): The laundry room was unlocked and had laundry detergent (1.5 gallons) and bleach (4 gallons). The Salon was unlocked and had two bottles of disinfectant wipes.
R432-270-22(8)(a)-(c): One employee did not have a current food handler's permit.
R432-270-25(1): Preventative maintenance was not being maintained according to a written schedule.
R432-270-25(5): The licensee did not ensure that hot water temperature delivered to public and resident care areas was maintained at temperatures between 105-120 degrees Fahrenheit.
R432-270-26(4)(a-k): Missing emergency preparedness for severe weather, explosions, bomb threat, and windstorms.
R432-270-26(6)(d)-(j): Emergency and disaster response plan missing instructions on how to contain a fire and use alarm systems, assignment of personnel to tasks, evacuation procedures, recruitment of additional help, delivery of essential care under emergency conditions.
R432-270-26(8)(a)-(d): There were no fire or disaster drills held in 2024.
R432-270-26(10)(a)-(g): Missing emergency lighting, heating equipment, and radio in emergency supplies.
R432-270-27(1)(a)-(d): One employee identified to work the night shift did not have all required first aid, CPR, Heimlich maneuver, and emergency procedure training.
R432-270-27(2)(a)-(c): Did not have a current first aid manual and clean-up kit for blood-borne pathogens.
R432-270-20(16): One resident's medications were not returned to the resident's responsible person with documentation upon discharge.
