Visit Reason
The inspection was conducted to review compliance with safety and care standards at Azalea Court Assisted Care Living Facility following allegations of resident safety failures and plan of care deficiencies.
Complaint Details
The visit was complaint-related, triggered by allegations of resident safety failures including a resident drinking a cleaner and another eloping and sustaining injuries. The allegations were substantiated based on interviews and record reviews.
