Bentley Manor, a 27‑bed residential care facility in Los Angeles, underwent a series of community‑care licensing reviews in July and early August 2022. The initial 7‑18 visit was a Component II telephone interview in which administrator Mona Alcaraz confirmed her knowledge of Title 22 requirements and the facility’s operating policies. The subsequent 7‑22 pre‑licensing inspection, conducted by Licensing Program Analysts Stephanie Cifuentes and Perry Scott, revealed several critical non‑compliances: the facility had only 26 beds, water temperature in 14 bathrooms fell below 105 °F, kitchen cabinets and drawers lacked locks for sharps and toxins, a number of rooms required repairs, and pests were present in the kitchen. The analysts issued a plan of correction, noting that licensure could not proceed until these deficiencies were addressed.

A follow‑up inspection on 7‑26 confirmed that a new bed had been installed, meeting the requested capacity of 27, and that many repair issues identified earlier were remedied. However, the water temperature problem persisted in the same 14 bathrooms, kitchen toxins remained unsecured, and pests were still detected in the kitchen. The analysts reiterated the outstanding items and set a new deadline of 8‑17 for completion of the remaining corrective actions.

On 8‑3, an unannounced visit by the same analyst found live pests in the kitchen, indicating that pest control was still inadequate. The facility’s compliance status remained incomplete, with the pre‑licensing evaluation still pending resolution of the noted deficiencies. Across the series of inspections, a pattern emerged: while administrative knowledge of regulatory requirements was established early, operational shortcomings—particularly related to physical plant safety, environmental control, and basic sanitation—persisted despite partial remediation. The licensing team has required ongoing monitoring and a clearly defined plan of correction, underscoring the importance of timely and comprehensive compliance to achieve licensure for Bentley Manor.
