Emergency call response: During the months of December 2023, June 2024 and April 2025, there were 277 e-call response times over the 7-minute limit, and documentation that the Residence monitor e-call response times over the seven-minute limit was missing for those months (repeat finding).

Resident screening and assessment: Two records were missing documentation of a current assessment noting that the Resident can independently navigate around a bed rail/ U-bar.

Quality assurance program: Documentation of the date the audit was completed was missing for the 2023 and 2024 calendar years reviewed, staff did not properly perform hand hygiene when providing SAMM medications, a loose pill was observed in medication storage units, a pill was given after falling on the floor without proper procedure, a medication administration record was missing from the medication storage unit.

Incident reporting: The Residence filed eight incident reports greater than 24 hours after the occurrence of the incident or accident.

Controlled medication handling: Care staff were observed not following the Residence’s Controlled Substance policies and procedures intended to prevent theft or diversion.

Resident progress notes: Documentation of significant occurrences in the Progress Notes was missing for two Residents.
