Visit Reason
Unannounced routine inspection to assess compliance with assisted living facility regulations.

Findings
The inspection identified 17 rule noncompliances including issues with staff identification badges, administrator duties, employee training, resident rights documentation, medication security, record protection, fire drill documentation, pet vaccinations, and maintenance safety.

Deficiencies (14)
R432-1-4. Identification Badges: One employee on duty did not have an identification badge.
R432-270-8(1)(b): The facility did not have a designee in writing to act as administrator when the administrator is unavailable.
R432-270-9(9)(j): Four employees did not have abuse and neglect reporting requirements in their annual inservice training.
R432-270-9(10): The facility administrator did not complete the minimum of four hours of core competency training including dementia and Alzheimer's specific training.
R432-270-9(14)(d)(ii): One employee did not have a tuberculosis skin test completed within two weeks of hire.
R432-270-10(2)(b): Resident rights did not include a statement that residents may file a complaint with the state long-term care ombudsman and other advocacy groups concerning abuse, neglect, or misappropriation of property.

R432-270-13(3)(a-b): One resident assessment incorrectly stated hospice status and assistance needs; service plan did not match assessment.

R432-270-23(5): Two 4-gallon containers of paint were observed in an unlocked utility room.
