Visit Reason
The inspection was conducted as a survey of the assisted care living facility Brookdale Columbia, resulting in findings of regulatory violations related to resident safety, dietary services, and facility cleanliness.
Findings
The survey found that three residents had eloped at least once, menus were not properly reviewed or approved by a Registered Dietician, food was improperly stored, the kitchen was unclean, and multiple residents' needs were not safely or effectively met by the facility.

Three residents eloped at least once, and care plans were not updated to reflect repeated exit-seeking behavior and the need for redirection. Facility staff failed to know when residents exited and did not confirm resident presence during rounds.
Facility menus were not properly signed and dated by a Registered Dietician, multiple menus were altered without RD review, and no consultant dietician was assigned as of July 13, 2023.
The food management company's last documented nutritional consult was in 2021, indicating lack of current dietetic oversight.
Several refrigerated food items lacked labels or open dates, dry food items were improperly stored including cheese snacks on the floor, and the stove and oven were coated with a thick, black, hard substance.
Multiple open dry goods bins had scoops left inside with flies present, and grime or residue was found in kitchen appliances including an electric mixer, toaster, microwave, and vent hood.
The facility permitted the continued stay of multiple residents whose needs could not be safely and effectively met by the facility.
