The complaint investigation was initiated on 2025-10-22 based on allegations of abuse by a certified medication aide (CMA #1) towards residents. The abuse was substantiated through eyewitness accounts. The facility delayed notifying families and did not notify physicians. The abuse incident occurred on 10/04-10/05/2025 but was not reported to the state until mid-week of 10/09/2025, violating reporting timelines.
