Visit Reason
The Office of the State Fire Marshal conducted a scheduled fire protection inspection at Riverside Senior Living to assess compliance with fire safety codes and maintenance requirements.
Findings
Multiple fire safety violations were identified, including unsecured electrical panels, improper use of power taps, missing fire sprinkler components, and carbon monoxide detector issues. All violations were corrected at the time of inspection or installed during the inspection.
Deficiencies (9)
IFC 603.2 2021 - Electrical panels (B2D1 and B2C1) were not locked, posing a hazard. Corrected at inspection.
IFC 603.5 2021 - Extension cord daisy chained into a multiplug adapter in room 324, and multiple multiplug adapters in use. Removed at inspection.
IFC 603.5 2021 - Refrigerator plugged into a powerstrip at memory care nurses station. Removed at inspection.
IFC 603.5 2021 - Penetration in the closet wall of the activity room was not properly repaired. Corrected at inspection.
IFC 705.2 2021 - 3rd floor sun room and memory care door had missing or improper opening protective assemblies. Corrected at inspection.
IFC 903.5 2021 - Missing fire sprinkler escutcheon in room 312 closet; extreme particulate buildup in 1st floor bathrooms and laundry; storage too close to fire sprinkler in kitchen pantry. Issues corrected or cleaned at inspection.
IFC 907.8 2021 - Missing monitoring module plate covers on the FACP in the memory care mechanical room. Installed at inspection.
IFC 915.1.3 2021 - Carbon monoxide detector required outside mechanical room on 2nd floor. Installed at inspection.
IFC 915.6 2021 WAC - Facility unable to provide documentation for Dec 25 and Jan 26 monthly carbon monoxide detector maintenance. Monthly inspections required per NFPA 720 8.7.1.
