Other violations were recorded in separate surveys: a single violation under Regulation Section 487.7 (f) (5) for resident services on February 8, 2021; a single violation under Regulation Section 487.4 (e) for admission standards on April 27, 2023; and a single violation under Regulation Section 485.11 (b) for records and reports on August 12, 2021. Each of these violations was addressed with a Plan/Notice of Correction that was approved by the Department.

The majority of inspections resulted in no violations. Zero‑violation surveys were issued on April 10, 2025; March 27, 2024; August 5, 2020; July 8, 2020; January 31, 2020; February 14, 2023; November 10, 2020; March 6, 2023; October 14, 2020; and May 26, 2022. These findings demonstrate a consistent pattern of compliance across the reporting period.

No enforcement actions were taken against the operator during the reporting period. The record indicates that all identified violations were resolved through corrective plans, and the facility maintained a high level of compliance with state regulations.
