Visit Reason
The inspection was conducted as an initial licensure survey for Canyons Senior Living, LLC.
Findings
The facility was found deficient in timely completion of 90-day assessments by the RN for six of nine sampled residents, and the medication refrigerator in the memory care unit was not maintained within the required temperature range, with 39 instances of temperatures below 38 degrees between 4/7/25 and 5/19/25.
