The facility failed to immediately report allegations that involved abuse neglect, exploitation or mistreatment, including injuries of unknown source or misappropriation of resident property to the administrator of the facility and to HHSC, but no later than two hours after the allegation is made, if the events that cause the allegation involve abuse, or result in serious bodily injury.

The facility failed to make sure the finishes on walls, ceilings and floors could be easily cleaned and sanitized.

The facility failed to make sure that surfaces on floors walls and ceilings in the kitchen are smooth and easily cleanable.
