Visit Reason
The Minnesota Department of Health conducted a survey to evaluate and assess compliance with state licensing statutes for an assisted living facility.

Findings
The facility was found to be in substantial compliance but had several deficiencies including infection control lapses, incomplete emergency preparedness plan, fire safety issues, incomplete resident contract information, incomplete resident assessments, and medication administration documentation errors.
