Inspection Report Summary for Cogir of Brentwood

The California Department of Social Services’ Community Care Licensing Division first visited COGIR of Brentwood on October 20, 2022 to complete a Component II compliance call. The administrator, Amanda Martino, demonstrated full understanding of Title 22 regulations and the facility’s operations. No deficiencies were recorded and the licensee was instructed to submit the signed LIC‑809 packet.

In January 2023, unannounced pre‑licensing and Component III inspections found the facility’s physical plant, staffing, and medication controls in acceptable condition. The analysts reported the facility was ready to be licensed, although final approval would still be pending at the Central Applications Bureau. The same day, a subsequent unannounced case‑management visit revealed an incident in which a resident’s jewelry was reported missing; a police report had been filed and the facility was conducting its own investigation.

A June 21, 2023 health‑checks visit, prompted by a priority complaint, uncovered a serious regulatory deficiency. The inspector observed that over‑the‑counter medications and cleaning supplies were stored within a resident’s bedroom who is under a Medication Management Program for dementia. This violates the California Code of Regulations on the care of persons with dementia, and a plan of correction was issued with a due date of June 22, 2023.

The most recent unannounced visit, December 14, 2023, addressed an incident in which a resident suffered an unwitnessed fall during a recreational activity, resulting in a hip fracture and subsequent surgery. The facility’s Health and Wellness Director confirmed the resident was now in a skilled nursing facility recovering and that a follow‑up evaluation would be conducted upon return.

Across the visits, a pattern emerges of a facility that is largely compliant with regulatory requirements but has recurring safety concerns involving resident belongings, medication access for vulnerable residents, and fall incidents. The licensing agency has documented each deficiency, issued corrective plans, and maintained an ongoing monitoring schedule to ensure resident safety and regulatory compliance.
