Deficiencies included medication storage and labeling, food procurement and sanitary practices, infection prevention and control, and administrative rules violations. Several deficiencies were corrected on revisit but some remained uncorrected.

Facility failed to protect a resident from physical abuse by staff. Video evidence confirmed rough and aggressive treatment. Staff was removed from schedule. Additional education and audits planned.
