The visit was triggered by a complaint alleging threatening behavior by a staff member toward a resident. The staff member was terminated. Additional follow-up may occur prior to a final determination.

No immediate health or safety risks were observed during the visit. The staff member involved in the incident was terminated by the facility.

The complaint alleged inadequate care and supervision leading to a resident wandering away during elevated temperatures. The allegation was unsubstantiated as the resident was confirmed to have the right to leave the facility unsupervised and no staff neglect was found.

The investigation found no evidence that staff neglected their responsibilities or failed to follow the resident’s care plan. The allegation was unsubstantiated based on interviews, records review, and direct observations.

The facility met all operational requirements for licensure, including fire clearance, physical plant safety, and adequate supplies. The inspection found the facility clean, in good repair, and hazard-free with all required certifications and safety equipment in place.

The applicant and administrator successfully completed Component II, demonstrating understanding of licensing laws and facility operation requirements. No deficiencies or violations were cited in this report.
