Inspection Report Summary for Ditmas Park Nursing & Rehab

During the reporting period, comprising 16 standard health citations and 18 Life Safety Code citations. These citations were identified across 12 on‑site inspections that resulted in citations, while 2 inspections concluded with no citations.
