Visit Reason
Surveyors conducted a complaint survey at Dominion Senior Living of Hixson on or about February 12, 2019, due to concerns about serious violations of state laws and regulations resulting in endangerment to the health, safety, and welfare of residents.
Complaint Details
The complaint investigation was substantiated based on observations that Resident #1 eloped from the secured memory care unit and the facility failed to prevent this, endangering the resident's safety.
Findings
The survey found that Resident #1, who resided in the secure locked memory care unit, eloped from the facility due to inadequate supervision. The facility failed to prevent Resident #1 from leaving the secured unit, violating required personal service provisions.
