A single case‑management visit in August 2022 addressed an elopement incident involving Resident #1. The staff documented a rapid response, the resident’s brief escape, and immediate recovery by law enforcement. Subsequent training on elopement recognition and consideration of additional exit safeguards were noted. The resident was subsequently relocated by the responsible party, and the investigation concluded without citing facility deficiencies.

Across the five inspections, census numbers fluctuated slightly—from 99 residents in July 2021 to 92 in May 2024—yet the facility’s capacity of 145 remained unchanged. The narrative consistently emphasized proper environmental conditions (temperature, lighting, hygiene) and an overall culture of compliance. No safety violations or complaints emerged during the period, and the facility’s ongoing adherence to licensing standards is evident from the documented inspections.
