Visit Reason
On-site inspection of Hospice Service Agency conducted 10 to 12 December 2024.
Findings
The inspection found one deficiency related to failure to implement accepted infection prevention practices during a home visit. Federal review found no deficiencies related to the Emergency Preparedness Program.
Deficiencies (1)
The agency failed to implement accepted standards of practice to prevent potential transmission of infections and communicable diseases during one of four home visits with a patient and CNA. This deficient practice had the potential to affect all 55 patients currently receiving hospice services.

Findings
No deficiencies were found during this unannounced onsite State Compliance survey. The Department approved the facility to continue operations as a Hospice Agency.
