The purpose of this visit was to conduct a compliance inspection.
Findings
The facility failed to ensure that portable fire extinguishers in the four transportation vans were maintained in operable condition, inspected annually by a qualified technician, and labeled with the condition and date of last inspection. The labeled tags on the fire extinguishers were missing.

The purpose of this visit was to conduct the annual inspection of Excell Adult Day Care Center.
Findings
The facility failed to ensure that all staff had current cardiopulmonary resuscitation (CPR) training, training in identifying elder abuse or self-neglect, training in diversity and cultural sensitivity, and training on Alzheimer's disease and other dementias for some staff. Additionally, the facility did not have satisfactory criminal record checks for some staff, lacked evidence of current nursing licenses, failed to ensure all staff had received physical exams within twelve months prior to employment, and did not have documentation of negative tuberculosis tests for some participants.
