Findings
The facility had multiple violations including failure to conduct employee background checks, incomplete resident service plans, inadequate tuberculosis screening, failure to report COVID-19 cases timely, and noncompliance with Life Safety Code requirements such as fire drills, emergency evacuation plans, and fire extinguisher maintenance.
